Please understand that this is only an estimate – not a guarantee – of what your insurance plan will cover. Your insurance company makes the final determination of benefits once a claim is processed. 

Because coverage varies by plan and patient, you are ultimately responsible for all charges incurred, including any portion not covered by your insurance. Payment for any remaining balance is due once your insurance company has issued payment.
